In Rearview: A Psychiatrist Reflects on Practice and Advocacy in a Time of Healthcare System Change Dr. Barry B. Perlman, a graduate of Yale Medical School, offers an overview of his career in medicine. Through recalled anecdotes Perlman brings readers along by writing about experiences from medical school and his psychiatric residency. He describes his first professional experiences with dying, his cadaver, and clinical rotations such as neurosurgery and psychiatry. He experienced the English National Health Service when taking his OB/GYN clerkship in London. Readers will be introduced to several of his dedicated professors and their eccentricities.Other chapters introduce topics central to the practice of psychiatry. They include consideration of suicide, violence, poverty, and electroconvulsive therapy. One chapter is revealingly illustrated with art done by patients and ponders the question of what makes art, art. Many of the chapters include interesting case presentations.Dr. Perlman, an activist psychiatrist, served as president of the New York State Psychiatric Association. He was appointed by NYS Governor George Pataki as chair of the NYS Mental Health services Council and to the State Hospital Review and Planning Council. Barry Perlman’s memoir, “Rearview, A Psychiatrist Reflects on Practice and Advocacy In an Time of Heathcare System Change” is an engaging and informative look at his long experience as a practicing psychiatrist, administrator, and patient advocate. Both erudite and humorous (you don’t see those words together often!), Barry vividly brings us into his professional life. Each chapter begins with a fragment, a memory of a past experience, followed by an elaboration of what happened and what he learned at the time and finally his afterthoughts on the matter given the perspective of time. It is a very effective narrative style. Barry and I were classmates at Yale Medical School and good friends. Although I eventually chose surgery over psychiatry, it is remarkable how similar our experiences have been: our commitment to our patients, our love of the science of medicine, the ethical responsibilities that a physician encounters in assuming the care of a patient, and the challenges that technical and administrative changes bring one over the course of a career. This is a great book for anyone interested in learning more about health care, and specifically mental health care, in the US and what it means to be a physician. Barbara Kinder, M.D.
